Our Diabetes Team will work with you, your family and your healthcare provider to help you live well with diabetes.

How this service works

We offer individual appointments and a wide variety of group programs for people living with diabetes to learn about:

– The basics of diabetes

– The impact of nutrition, exercise, stress, pain, sleep on blood sugars

– The importance of regular foot and eye screenings

– Medication initiation and adjustments (as recommended by your doctor or nurse practitioner)

Group learning sessions are offered monthly for anyone with prediabetes the 1st Tuesday of each month from 5:00-6:30pm

Group medical visits are offered quarterly (instead of 1:1 sessions) to review your diabetes lab results + tips to maintain:

  • Healthy blood sugar

  • Healthy blood pressure

  • Healthy cholesterol levels
