Country Roads Community Health Centre is committed to promoting risk identification and management practices that supports quality programs and services.

Country Roads Community Health Centre has adopted an Integrated Risk Management (IRM) Program to proactively manage and analyze risk as part of the ability of the organization to achieve its strategic goals and objectives. This takes place within a culture of continuous quality improvement, where issues are identified and resolved in an effective and systematic manner.

Quality Improvement Plan

A Quality Improvement Plan is a set of commitments to improve specific quality issues made by Country Roads Community Health Centre to its community. A Quality Improvement Plan is developed, documented, and made available to the public annually by health care organizations in Ontario. The goal of the Quality Improvement Plan program is to drive improvements across multiple sectors of the health care system that address province-wide priority issues – advancing equity, inclusion, and diversity and addressing racism – to achieve better outcomes for all people in Ontario through the cumulative efforts of every individual organization.
