Our clinical pharmacist works closely with your primary care team to help make sure that your medications are working well and safely for you. They are available on-site to help you with managing your medications and answering your medication-related questions.
How to access service
If you would like to speak with the pharmacist, simply ask-at this time you do not need a referral from your health care provider.

We strongly recommend that you meet with our pharmacist for a comprehensive medication review if
You have been recently discharged from hospital or a rehab facility
You are taking more than 5 medications on a daily basis
You are taking higher risk medications such as opioids, benzodiazepines, blood thinners, and insulin
You or someone you know have concerns about your medications or your health status has changed
What is a comprehensive medication review and what can I expect?
A comprehensive medication review is a review of ALL medications that you are taking and how you are taking them. The pharmacist will ask you a number of questions to help assess that the medications you are taking are still needed and are working well for you. The initial appointment with the pharmacist will last approximately 1 hour. If needed, a follow-up appointment can be booked.
How can I prepare for my appointment with the pharmacist?
Please bring ALL medications that you are taking. This includes medications that require a prescription, ones that can be purchased over-the-counter, vitamins and supplements. Do not forget about things like eye drops, creams, inhalers, patches,
Write down any questions or concerns you have about your medications so you can be sure to have them addressed during the appointment
What should I expect to take home at the end of the appointment?
By the end of the appointment, you will also be able to:
Understand WHAT each medication is for and WHO has prescribed it
Understand HOW each medication should be taken
Understand the RISKS and BENEFITS of each medication
Understand IF and WHEN a medication should be CHANGED or STOPPED
