A client navigator helps a patient

People are healthier when they feel connected and are linked with social supports and resources in their communities. The Client Navigator at Country Roads can assist you with finding services and resources in your community that meet your healthcare needs.

They will work with you to coordinate your care, in order to help you stay as well and independent as possible. As housing, income, social support, education and employment can greatly impact your overall health, the Client Navigator can also support you in these areas as well.

How to access this service

Ask your health care provider, a member of our nursing team, or the admin staff about setting up an appointment.
