Join our small, tight-knit, and dynamic, 45-person team at Country Roads Community Health Centre (CRCHC) located in the small towns of Portland and Westport, at the heart of cottage country along the historic Rideau Canal.

Key Responsibilities

  • Provision of comprehensive, coordinated health care encompassing primary care, illness prevention and health promotion

  • Manage a roster of a maximum of 1137 primary care clients

  • Working in a collaborative environment with a diverse interprofessional team of other physicians, and nurse practitioners

  • Collaborate with our team of professionals including social worker, dietitian, pharmacist, respiratory therapist, diabetes educators, chiropodist, nursing, and community health workers

  • Shared on-call services amongst primary care provider team

  • Incorporating a health promotion approach, addressing the social determinants of health
